On-Site Health Risk Assessments

Our wellness team provides on-site health risk assessments to evaluate the overall health of your employees and to help reduce health care expenses. Our wellness team will travel to you, making the process easy and convenient. All we need is a space, tables and chairs. We will bring our supplies and privacy screens. We will set up a registration table, a blood pressure station, body composition station and a blood draw/lab station.

• Individuals will schedule their own appointment times for the on-site assessment through the online wellness portal.
• Individual results will be uploaded to their online wellness portal. This portal is secure and HIPAA compliant.

Our wellness program offers the Total Wellness Portal and mobile app, making it easy for your team to connect and participate. Members can also sync a wearable device, such as a Fitbit watch. The portal also provides access to a personal health questionnaire, which determines health risk factors for chronic disease. 

Once all the biometric measurements and lab results are completed, our team will help identify top health risks and create a wellness program suited just for your population.
